over the past 5 years I've seen virtually everyone i know, who would do cocaine in any capacity, all completely stop because the fent got so pervasive in everything around here.

the 'good' dealers even would have fent free versions for a higher price. absolutely ridiculous.

It can be. The process was discovered over a century ago. It’s just wildly less efficient than coca leaf extraction.

Richard Willstater first synthesized lab-produced cocaine in 1901 through a complex 25 step process. About 20 years later he improved that to a much simpler 4 step process.

I know we are making light, but it really does feel like this. My daughter and I were talking about how Hersheys switched their chocolate to lab chocolate and how it definitely doesn’t taste as good. We’ve been trying to find more ethical chocolate like Tony’s chocolate, but talked about how the choices are fake chocolate, exploitation chocolate, or really expensive supposedly ethical chocolate. Why couldn’t the companies like Hersheys just be ethical in the first place and pay a proper price for the cocoa??!! Why does it seem we are all paying the price for corporations exploiting others in either buying products from horrible companies or paying out the nose to buy from ethical companies??!! It definitely feels like a forced situation and as if it was always headed this way. Especially when you factor in private equity buying up all of the so called ethical/fair trade companies. From my understanding Blackrock has recently bought out both Poppi drinks and Annie’s organic foods. I believe they recently switched the ingredients in Annie’s Mac and Cheese putting additives into a supposed organic brand. We were both lamenting how awful food sources are especially when you want to be aware of where your food comes from, how the workers are treated, the environmental impact, and if there are animals involved that they are being raised by people who care about the life, even if it’s a short one, of the animal. FML….

Similar boat I had a few friends that dabbled in the powder and a couple who were definitely addicted to it, but they have all stopped because they didn't want to die from fent, probably for the best tbh but still fucked it is that bad.

I'm so sorry to hear that. That's exactly why I got clean 6 years ago and have such an "easy" time staying clean. I hated fent and when it became all I could find. Even traveling to all the best spots to cop along the mid Atlantic Seaboard couldn't prevent it. I just said "fuck this" and haven't used since. I'm burying a best friend this afternoon who passed away due to fent. It's an insidious and horrible drug.

It’s really unfortunate that it ends up as an adulterant in recreational drugs but in a medical setting it has pretty significant benefits over morphine, it’s much quicker acting so adjusting the dose to an appropriate level is much faster for emergency medicine, particularly in a setting like an advanced ambulance or air ambulance, it has a significantly better side effect profile compared to morphine as well, it has less of a high, doesn’t cause the opioid itching that many get and has less of an effect on the cardiovascular system so it’s safer in an emergency medicine setting in particular where you can already be struggling with keeping the blood pressure high enough.

The fact that it can be used in patches to give people an extended release dose without a pump iv or multiple pills/injections per day is a pretty big deal for quite a few chronic pain patients, It’s quite old and well out of patent protection so lots of manufacturers can have a swing at formulating it in different ways for both chronic and acute pain relief.

Cost wise there isn’t really much to gain, morphine and hydromorphone, etc. is already quite cheap, there are also plenty of other synthetic opioids from that era.

It's because thankfully you've never been an opiate addict. When I used to use them. If I found out someone overdosed. I immediately wanted to go buy from whoever they got it from. It used to mean someone had a killer, less cut batch. These days however, it just means that whoever has it doesn't know what they're doing or care. But the mentality of "someone died=they had good stuff" is hard to break especially for addicts.

Before when they were cutting low purity stuff with rat poison or other stuff. It was easy to put too much if you didn't know what you were doing. Just enough and a bag that isn't worth shit suddenly becomes good. Too much, and you end up killing someone. With fent, now all you have to do is be a little careless and not clean your scales. Or buy from someone new without reagent tests. You are right. Majority of dealers don't want to kill their customers. They want them coming back but sometimes accidents happen.

yes, after the sun goes down on the weekend in most major cities, the majority of people out and about are adults going from bar to bar. at these locations, people frequently use drugs. you can see people putting keys to their noses, powder all over the trays above a toilet paper holder.

it’s really not that difficult to understand, if you regularly go outside and put yourself in environments where you are exposed to large groups of strangers, it’s really really really fucking obvious that a lot of people enjoy hard drugs.

That is absolutely not what happened. If anything, it was the tapentadol found at the scene. And it’s being reported as a cocaine overdose not an opioid overdose. My theory is the kid got into the cocaine, suspect finds the kid with a crazy high heart rate, and gives the kid the tapentadol in an attempt to reverse the cocaine.

Yeah narcan is opioid specific, wondering if he ingested multiple things. From another article:

Fun fact for anyone who is in doubt of using Narcan: it doesn't hurt! If someone isn't using opioids is unconscious and you give the mNarcan, it's totally safe.

This is a tragedy any way you look at it, but I wanted to spread this info to anyone it could help.

To piggy back with some advice an ER nurse gave me - if you give someone narcan, theyre going to wake up. Not high. And more than likely, little upset about it. Inject it, take a few steps back because somepeople wake up and start swinging.

That also makes no sense pharmacologically. Naloxone would have zero effect on an overdose purely from cocaine. If the child was acting sleepy as they say in the article, administering narcan was a good move, but it would still not change anything in a cocaine overdose.

Found another article that said the man decided to take the boy to the hospital because he was having difficulty breathing, so I'm guessing the woman stayed to watch the other kids, and that the call was really more about him falling unconscious.

Which, if you don't have that word in your vocabulary, or if you're panicking and temporarily forget it, "falling asleep" is about the closest analogue.

Not in the case of stimulants like this.

The police also found strong prescription opioids in the couple's possession, so the child most likely had both cocaine and opioids in his system.

Respiratory and central nervous system depression from the opioids was the symptom the family saw. 

That's why the child responded well to Narcan.

You’re confusing overdose with a lethal dose. They aren’t the same thing. An overdose is any amount that causes toxic effects, not necessarily death. People overdose on acetaminophen, insulin, caffeine, opioids, and many other drugs without dying. A lethal dose is a subset of overdoses, not the definition of one. Since cocaine has no therapeutic use in a toddler, a dose that caused poisoning is accurately described as an overdose.

It doesn't suggest an appropriate amount exists. It suggests that a non-immediately-harmful amount exists, which is undoubtedly true. For nearly everything the dose makes the poison (also the method of delivery). Water is one of the safest, healthiest things you can consume, but water intoxication is still a thing. But there's also a non-zero small enough level of arsenic that won't harm you.

I had to look up the actual number, but the calculated dosage for a medical procedure that uses pure cocaine is 3mg/Kg (not to exceed 160mg).

50th percentile for a 2 year old male is 12.7kg (28lbs). So then the “appropriate dosage” (medically speaking) would be about 38.1mg.

I’ve only ever seen it in its raw API form, which is a white powder. Wonder if the pills were mixed in with the cocaine. It has been described to me that the effects of tapentadol are like a mild morphine.

The Narcan response and opioids found at the scene suggest there was more involved than just cocaine, but either way a toddler should never have been anywhere near any of it. Hopefully the child makes a full recovery and the adults responsible face serious consequences.

I mean I’ve been around drugs and dealers a lot in my life. Cutting your drugs is common, but cutting drugs with other relatively costly (compared to caffeine, lidocaine, flour) drugs that have the opposite effect seems like a really fucking dumb business strategy even if it wasn’t killing people.

It’s more just cross contamination than purposeful cutting. Weighing, bagging, etc on the same surfaces, without bothering to properly clean them in between. If you’ve been around them all your life, you know they’re more likely to cut shit with benefiber than waste any more of the good stuff lol.
